Start with the hazards, not a form. Work through the 17 recognised psychosocial hazards, note which ones apply to your workplace, then assess each one properly rather than trying to cover everything in a single generic survey.
A survey tells you where perceptions of risk are highest, but it doesn't give you a case file, an action plan, or evidence of consultation and follow-up — the parts regulators actually check. Treat a survey as the input, not the whole assessment.
A documented hazard register, risk ratings, controls, an action plan with owners and dates, evidence workers were consulted, and a review record. Missing any one of these is a common finding in enforcement cases.
